Forms

The following are forms required for the 2011-2012 school year. Please click on the appropriate grade level for a form checklist and individual forms.

Other Forms That May Be Need Throughout the School Year:

  • Asthma Action Plan: This form, to be filled out by your doctor, is required if your child has asthma.
  • Food Allergy Action Plan: This form, to be filled out by your doctor, is required if your child has a food allergy.
  • Field Trip Medical Form: This form gives permission for your child to take over the counter or prescription medicine while on a field trip.
  • Temporary Medical Power of Attorney Form: This form MUST be filled out for each field trip your child goes on if you do not sign the Power of Attorney form at the beginning of the school year.
  • Permission to Dispense Over the Counter Medicine: This form gives permission for our staff to administer over the counter medicine to your child during the school day on a short-term basis (up to two days). If your child needs over the counter medicine administered on a long-term basis, you must have a physician's order.
  • Request to Administer Medicine at School: This form, which needs to be filled out by both a parent and a physician, gives our staff permission to administer over the counter or prescription medicine on a long-term basis (more than two days) while at school.
  • Permission to Walk/Bike to School: This form gives your student permission to walk or bike to school.

  • Middle School Sports Registration Forms

    • Fall: As of now, Cross Country will be the only sport offered to Fifth Grade students. If space allows, other sports may be opened to Fifth Grade students in the early Fall.
